There have been reports of unintended acceleration of Toyotas, mostly in the USA where Toyota has recalled vehicles. Toyota Australia say the US situation can't occur here because the cars sold here have a different accelerator arrangement that precludes it happening here. Toyota recalled the RAV4 twice in the US for "unintended acceleration" problems, which sounds uncannily like your problem. The first recall was, the company claimed, because the carpet could move forward and over the accelerator, the second was because the accelerator pedal could "stick". There was also some suspicion that there was also a problem with the 'fly-by-wire' throttle, but nothing has been found on that front. I would urge you to report the problem to Toyota directly and ask that they inspect your car.

Rust isn't a particular issue with the Prius any more than it is on any other Toyota. I would show it to your dealer and have Toyota inspect it with a view to repairing it.
If the shuddering isn't getting worse I would probably persist with it until it starts to slip. If it slips, then definitely replace it.
Checking and adjusting the valve clearances is a part of the service schedule on the Toyota diesel, and the parts do wear out. This is a case of buyer beware, you really need to check the fine print on the warranty to find out what isn't covered before you actually spend your money.
It could be related to the transmission calibration, but it's not possible to say for sure without driving it. Take it back to your dealer and take him for a drive so you can demonstrate the problem. That way he might be able to fix it.
