Toyota won't fix what I think is a common fault covered by warranty on my Prado. What should I do?

Our 2010 Toyota Prado blew out a puff of smoke and then lost power on our return from holidays. It has done 166,000 km and is still under extended warranty having bought it from a Toyota dealer two years ago. We had every service done by the same Toyota dealer, including one a few days before our trip. The first diagnosis blamed a fuel injector drilling a hole through the piston resulting in engine failure. Since I challenged this as "a known fault" and therefore we reasonably expected the dealer or TMC to pay for it to be fixed I got this response: "We have inspected the injectors and found no signs of carbon build up or deposits, or any signs of excessive carbon build up through the injector jacket and lubrication system. We have also tested the injectors and found them to be serviceable, with no signs of failure. The only signs of failure are isolated to the damaged piston, and residual damage as a consequence of its debris travelling through the engine."My research suggests that this too is "a known fault." We reasonably expected a vehicle of this quality and price to last longer than two years. Can please you advise?

This is typical of a problem with the Prado blowing engines at high mileage. It was due to carbon buildup on the oil pickup in the sump, the carbon resulting from a leaking fuel-injector seal that allowed fuel into the oil and contaminate it. The dealer's report would suggest that this is not the case, as they appear to have checked the fuel and oil systems for contamination and carbon buildup. You could have an independent mechanic check the engine for you to see if they could find a cause that contradicts the dealer's findings.
